Ask for a photo when the fault is something you can see

A picture of the torn pad saves a visit. A picture of a black screen does not. Ask only when it helps.
Not every fault is visible. A treadmill that trips a breaker looks like a treadmill. A bench with a torn cover does not need a paragraph.
If the member can photograph the damage, the missing part, or the error text, ask for that. If the problem is a feeling under load, do not insist on a photo of a normal-looking machine. You will get a blurry picture of the floor.
What the photo is for
It helps the next person bring the right part, or decide the machine stays open. It is not evidence in a courtroom. A clear enough image attached to the fault is the whole requirement.
Store it on the fault, and copy it to the job if the technician needs it in the field. A photo that only lives on a phone is the same as a photo you do not have.
When the job is done, the after photo belongs with the work order. The original complaint photo stays on the fault, so you can see what was reported rather than what you hoped was reported.

The useful sentence is whether they can use it, and whether you already know. The part number is not for the floor.
Reset the rig to a known plan after class
A rig left as the last workout set it is a different machine. The closer puts it back and notes what broke.
Find the model and serial number on gym equipment
The model and serial are on a plate on the frame or inside the console bay. Photograph them. If the plate is gone, say so. Do not invent a number.
