Anchors and wall fixes are safety checks, not decoration

A rig, a rack, or a wall ball target that moves is already a fault. Tightening it ‘when we have time’ is the wrong status.
If it is supposed to be fixed and it moves, take the related station out of use until you know why. Movement means the anchor, the bolt, or the wall is no longer doing its job.
Do not keep tightening a spinning bolt and calling it maintained. If it will not hold torque, the fix is a proper anchor or a competent look at the substrate. Write what you found: hollow block, cracked concrete, a bolt in the wrong hole.
After any move
Re-check the same day. A rack that was safe in the old corner is not automatically safe in the new one. The floor may be different. The fixings may have been bent on the way.
Wall-mounted kit used by classes needs a named person on the inspection, not “the coaches keep an eye on it”. Eyes are not a record.
A missing bolt is a fault even if the frame still stands. The next missing bolt is how it stops standing.

Smaller does not mean informal. A broken hold, a loose bolt, or a torn mat is a fault with a status.
Clearance is a condition of the machine
A treadmill too close to a mirror, or a rack too close to a walkway, will be reported as the machine’s fault. Measure it once.
A hot smell is a stop, not a note for later
Hot insulation, a warm plug, or a console that smells sweet and sharp: take the machine off and do not switch it back on to ‘see’.
