A failed inspection has to create work

Fail, then nothing, is how the same note appears next month in a different handwriting.
An inspection that can fail on paper and change nothing on the floor is a diary. When a line fails, open a fault or a job before you finish the inspection. If the failure is unsafe, change the status and hang the sign in the same visit.
The note, the photo, and the name of the inspector stay on the machine. “Failed by person unknown” is not a record you can defend or learn from.
Follow-up is a status
Follow-up means someone owes a look, not that the problem is mild. Give it a job with a date. If the date passes, it is overdue, not forgotten-by-design.
Do not let a later pass erase the fail. The history should show it failed, then what closed it. Deleting the fail to tidy the month is how a pattern disappears.
If you disagree with the fail, write why and what you checked. Do not silently change it to a pass.

Kit in a children’s area is still equipment
Smaller does not mean informal. A broken hold, a loose bolt, or a torn mat is a fault with a status.
Clearance is a condition of the machine
A treadmill too close to a mirror, or a rack too close to a walkway, will be reported as the machine’s fault. Measure it once.
A hot smell is a stop, not a note for later
Hot insulation, a warm plug, or a console that smells sweet and sharp: take the machine off and do not switch it back on to ‘see’.
